What is it?
As a ‘Thank You’ to Advanced Family Eye Care’s loyal patients for choosing our team and referring new patients, a $1,000 cash drawing will take place at the end of February 2021.
How to enter
- Refer a new patient(s) to Advanced Family Eye Care’s Waterloo or Independence location.
- New patient must complete a full eye exam prior to February 28, 2021 for the entry to be active.
- All active patients having received a full eye exam since December 1, 2018 are eligible.
- Each new referred patient will earn an entry. There are no entry limits.
- DON’T FORGET! Be sure to have family and friends mention your name when calling to schedule an eye exam!
